    Assessment of prescribing pattern and cost analysis of topical steroids for skin disorders in dermatological outpatient department of a tertiary care hospital

    Background: Topical steroid is most commonly prescribed in non-infective dermatological conditions. Periodical Prescription audit is mandatory for the effective management. Hence the present study is designed to assess the prescribing pattern and cost analysis of topical steroids for various skin disorders in the dermatology OPD of a teaching hospital.Methods: This is a prospective observational study conducted in dermatology OPD of a teaching hospital from Jan-Mar 2019 in patients prescribed with topical steroids in all age groups after obtaining IEC approval. Data was analyzed for prescription pattern and cost analysis using descriptive statistics and expressed in percentage.Results: A total of 90 prescriptions were analyzed among which males were 45% and females were 55%, the common indications were eczema-27.7%, atopic dermatitis-25.5%, psoriasis-16.6%, dermatoses-13.3%, lichen planus-7.77% etc. Topical steroids commonly prescribed were super potent (Clobetasol 34.4% and Halobetasol 22.2%). Cream and ointment formulation were commonly used. Common adverse reactions were skin atrophy, hypopigmentation, acne. In prescribing pattern, specification of strength and quantity were lacking whereas instructions regarding area of application-44.4%, route of administration-83.3%, frequency and duration-91% were noted. In cost analysis, comparison is made between similar potency and clinical outcome. Of which, Clobetasol 0.05% and Betamethasone 0.01% is found to be cost effective compared to Halobetasol 0.05% and Mometasone 0.1% respectively.Conclusions: This study provides a limelight on prescribing pattern of topical steroids and emphasize periodic audit to rationalize the prescription with cost effectiveness

    Rationality of the use of factors of production by small trawlers along Calicut coast

    The purpose of the present study is to evaluate the economic rationale of input use by small trawlers in Kerala Coast. In order to estimate the production elasticities of different inputs, a Cobb- Douglas type of production function was fitted to the pooled data on coast and earnings of 50 trawlers for a period of 10 years from 1971-72 to 1980-81, obtained from Kozhikode Regional Fish Marketing Co-operative Society. For functional analysis, vessel-wise gross returns (in rupees) was taken as dependent variable and number of days fished, fuel expenditure (in rupees) and repairing and maintenance charges (in rupees) as independent variables. Marginality analysis was also done to evaluate the eflSciency of input use. The production function as well as the marginality analysis indicated that the increase in the number of days fished would lead to higher gross income. So also increase in oil expenditure from mean level would yield a more than proportionate increase in gross income. However, repairing and maintenance cost had a negative impact on fishing income which indicated that the vessel owner in his own interest should be cautious in the operation to avoid frequent repairs and to reduce the delay in getting the vessels repaired

    Correlation of clinical and radiological outcome of distal radius fractures treated with volar plating

    Background: Distal radius fractures are one of the most common injuries treated by an orthopaedic surgeon, accounting for approximately one sixth of all fractures. Due to increase in incidence of high velocity trauma, the injury is seen occurring in young patients more often. A displaced fracture deranges the wrist anatomy causing deformity and loss of function at the wrist joint. Open reduction and internal fixation with plate using a volar approach allows anatomical reduction of the fracture, stable fixation aiding in early mobilization.Methods: Our study was a hospital-based study conducted during the period June 2016 to November 2018. A total of 30 patients with distal radius fracture were treated with ORIF with a volar plate. They were followed up for a year and clinical and radiological outcomes were evaluated.Results: Among the 30 patients studied 25 were males and 5 were females with mean age of 36.23 years. According to Gartland and Werley demerit scoring system, 15 of them had excellent result at the end of one year. 13 Of them had good and 2 of them had fair results. Radiological assessment was done by SarmientoтАЩs modification of the Lind storm criteria and 15 of them had excellent results. 15 patients had good radiological outcome.Conclusions: From this study we conclude that ORIF with volar plating can provide good clinical and functional outcome in distal radius fractures. Anatomical reduction of the fracture fragments is the key in achieving good results

    Prawn farming is a potential area of income generation for women in fishing villages. Observations indicate that women in small scale prawn farming households do have involvement in the traditional system. They also play an important role in the management of other activities such as livestock rearing and vegetable cultivation in their farms. Given more information and training they can be usefully involved in the adoption of the technology and also management of the farms
